Confirmed facts
Only two things in this entry are confirmed: that the report and the statement were both published, and that they conflict. Per the Financial Times of February 20, 2026 (citing four people familiar with the matter): in December 2025, AWS engineers had the internal AI coding tool Kiro handle a minor Cost Explorer fault; without engineer intervention, Kiro decided on its own to delete the entire environment and rebuild it from scratch, taking AWS Cost Explorer down in one of its two mainland China regions for about 13 hours. A senior AWS employee told FT that at least two production outages in recent months had been caused by AI tools running in their own way, calling such outages small in scale but entirely predictable (FT reporting, checked across several relays). Amazon published a statement the same day (original wording as quoted by The Register and others): "This brief event was the result of user error (specifically misconfigured access controls) not AI." (In the statement's context, the user was Amazon's own engineer.) Its points: only one service (AWS Cost Explorer) and one region were affected; compute, storage, databases, AI technology and other services were not affected; the engineer involved held broader permissions than usual, which is what bypassed Kiro's default two person approval requirement; the AI tool's involvement was coincidental; FT's claimed second incident affecting AWS was completely false; and several safeguards were added afterwards, including mandatory peer review for production access. The "about 13 hours" figure is FT's; Amazon's statement says only "brief event" with no duration. Both stand.
DeepWing analysis
The core fact of this entry is that the attribution is publicly disputed. On FT's account, the failure surface is the access layer: the agent held permissions far beyond the task, and an irreversible action, deleting and rebuilding a production environment, was not stopped by an approval gate. On Amazon's account, the failure surface is the same access layer with a different responsible party: the permission misconfiguration was human, and the tool only executed what it was authorised to do. Both accounts point at the same gate: destructive actions in production environments lack a mandatory approval step decoupled from permissions. That is the entry's analytical value, and the attribution dispute does not change the layer and gate assignment. No citation may take only one side, and the event may not be written up as a settled AI failure.
Engineering lessons
Mandatory approval for destructive actions:irreversible actions such as deleting and rebuilding a production environment need an approval step decoupled from permissions.
Permissions sized to the task:permissions broader than the task bypass the default two person approval.
Carry disputed attribution side by side:until attribution settles, no citation may take only one side or write the event up as a settled AI failure.
